Health history intake & consent forms must be completed prior to treatment to meet legal and professional standards. All forms will be sent via email once patient profile is created. 

Service prices are before tax (HST 13%).

Seniors (65 and over) & youth (12 & under) 20% off.

A co-payment may be required depending on your insurance plan when using direct billing.
